Online Award Recommendation Form

Welcome to the online Calontir Award Recommendation form.

All members of Calontir are encouraged to send in award recommendations for those members of the populace that they believe are worthy of recognition.

You may wish to see Awards of the Kingdom of Calontir. It includes a list of awards given in Calontir.

Please review the online Order of Precedence before making an award recommendation. It is helpful to help make sure your recommendation has not already been conferred.

You have two options for submitting your award recommendation:

  • Print out a blank Award Recommendation form. Please fill it out and and deliver it to Their Majesties.

  • Send your award recommendation to Their Majesties directly via e-mail by filling out the online form below. Please complete all fields, then press the “submit form” button.

Labels that are bold and marked with form required field marker indicate a required field.

Please note that the save and recover feature of this form requires JavaScript. A small cookie (4KB encoded) will be stored on your local machine. The Kingdom of Calontir Web site does not collect or store your form information.

The save and recover feature will work with dynamically added form fields. However, the number of fields saved and recovered must be the same in order for the recovered data to be populated correctly. (E.g., if you added two form fields to your form when you saved your information you will need to add two form fields when you recover it.)

Where available, the preview feature of a Web form requires JavaScript. The preview page will load in a new tab or window.
